Using custom classes in a form loses object information

Description

I created a simple form and added a toolbar from a saved class. Previewing the toolbar works fine. Hovever, as soon as I run the form, the labels and bitmaps do not show.

The two files you attached were identical copies of the form. Could you also include a copy of your toolbar class? In the form it's referenced as 'bcToolbar.cdxml'
